Title: General factors involved in implant osseointegration – Dr. Riță George-Marius, Dr. Sica D.C.Diana-Florentina, As. Univ Dr. Tirban Pantelimon

Dr. Riță George-Marius is a dentist, general dentistry resident year I, at the Arad University Center. He is a graduate of the Faculty of Medicine Lucian Blaga Sibiu, class of 2023. He is a doctor who is proficient in implantology and implant-prototics, through close collaboration with a specialist surgery O.M.F.

General factors have a strong influence on the success or failure of dental implant insertion. However, they are frequently overlooked, either by the clinician or by the patient, who does not realize their importance or does not provide complete and conclusive information about the conditions they suffer from and the treatments they are undergoing.

The objective of this paper is to present some of the most important general factors, both theoretically and through clinical cases.

According to international studies, the average success rate of insertion and osseointegration is over 98% in more than 1,200,000 implants inserted, but this percentage can drop significantly when there are associated general conditions such as diabetes mellitus, heart disease, smoking and chronic anticoagulant therapy.

In diabetes mellitus, osseointegration is impaired by inhibition of osteoblast differentiation, altered action of parathyroid hormone (PTH), deleterious effect on the bone matrix and its components, affecting adhesion, growth and accumulation of the extracellular matrix.

In heart disease, osseointegration is impaired by reduced peripheral blood supply, insufficient tissue oxygenation and impaired healing.

In conclusion, for successful treatment, it is necessary to keep the general symptomatology under control and to adopt a case-specific therapeutic approach.
